Make an appointment or call us at 321-639-2020.

As a courtesy to our patients, we make every effort to run on time.  In order for us to continually run on time, we recommend all new patients download, fill out and bring in our patient forms when they arrive for their appointment.  Otherwise, we recommend arriving 10-15 minutes prior to your appointment to fill out the forms. When coming for your visit, please have a photo ID and your insurance card for validation.

We highly recommend making routine and non-emergency appointments in advance. Walk-in patients will be accepted if time is available.  If you have a medical concern or emergency during office hours, call us as soon as possible and we will try our best to see you quickly.  We reserve the right to reschedule or cancel patients who arrive 10 minutes or more later than their scheduled time.  If you are running late, please notify our office as soon as possible.

Please give our offices 24 hours advanced notice if you need to cancel or reschedule, so we may open up your time slot to other patients.

Insurances

We submit claims to insurance companies with which we have a contract. It is the insured’s responsibility to know the type of insurance, coverages and any applicable copays or deductibles.  Please verify with your insurance that our Doctors are participating providers. If insurance is denied or reduced for any reason, you are responsible for the remaining balance.  Do not hesitate to contact your insurance company if you disagree with their payment or to find out the status of your claim. We reserve the right to not do an examination on a patient with any unpaid balances. 

It is our policy that full payment is due at the time services are rendered.
